    Employee Self Reporting COVID-19 Form

    If you answered yes to any of the questions on the Campus Clear app for daily symptom checking OR are experiencing COVID-19 symptoms OR have been exposed to someone with a confirmed case of  COVID-19, please complete the COVID-19 Self Reporting Sickness or Exposure Form below. Once the form is received, further instructions and information will be given to the employee by the Benefits Office. All medical information shared is kept confidential.

    Employee Accommodation Request due to COVID-19 Form

    If an employee has an underlying medical condition that puts them at higher risk for severe illness with COVID-19 as defined by the CDC and the employee seeks to obtain a formal accommodation, please fill out the below form. Any medical information shared will be kept confidential and stored in the Benefits Office.
