Secured Placement Form
A 'Secured Placement' is a placement to observe another teacher in your own school. It can not be used to observe a classroom in which you now or have previously worked in. A 'Secured Placement' may only be used once. Fill out the form completely. Get signatures from appropriate personnel, sign and date. Save a copy of this form for your records. Return the original hard copy to the Office of Clinical Experience by the 10th day of the semester.

Background Check Information
Every teacher candidate must have a background check done for the school district they are placed in. It is your responsibility to complete, pay for and submit all forms needed to the appropriate district personnel BEFORE you begin your placement. If a background check is not submitted to the school district before your first day of student teaching, it may result in a pulled placement.
