For your convenience, any forms not requiring a signature are submittable online and will be automatically forwarded to the appropriate staff member for processing.  A copy will be automatically forwarded to the sender for his or her records.

Address Change - The fastest way to change your address is online via Piperline.  If you cannot access Piperline, please use this form. 

Name Change - Legal name and gender changes can be made, with appropriate supporting documentation, via this form.

Academic Advising Worksheet 

Course Substitution and Prior Approval - Use this form to gain approval for courses you plan to take or have previously at another institution to count toward your degree. 

Declaration of Major/Minor/Certificate and Change of Advisor  

Flexible Curriculum Proposal - A student may design his or her own major with approval of this form.  Must be submitted no sooner than the first term of the sophomore year and no later than the first term of the senior year. 

Grade Appeal Request Form - Students may appeal the grade assigned to them in a course.  Submission of an appeal does not guarantee a grade change.  

Undergraduate Petition of Academic Policy - Students may request an exception to academic policy by submitting a petition to the Academic Affairs Committee. Submission of a petition does not guarantee approval.  

Undergraduate Request for Incomplete Grade - Students may work with their instructors to contract for an incomplete grade in a course.

 Summer 2013 Undergraduate non-Hamline Visitor Registration Form - Visiting students use this form to register for Summer 2013 undergraduate courses.  Summer registration opens Monday, February 11th, 2013.

Cross-School/Cross-Program Registration - Complete this form to register for a course outside of your school or program. 

ACTC Cross-Registration - Undergraduate form to request registration at one of Hamline's ACTC partner schools. 

Undergraduate Add/Drop/Withdrawal 

Apprentice Teaching Contract - Students may earn up to 4 apprentice teaching credits, graded on the Pass/No Pass system, by assisting faculty with their teaching responsibilities. 

Undergraduate Audit Registration - Register for a course on a 0-credit, ungraded basis for a flat fee.  This form is also for visiting Amity Scholars. 

Collaborative Research Registration - Collaborative research projects offer students the chance to pursue independent study in greater depth with a faculty member, often in conjunction with the professor’s own research. 

Departmental Honors Project Proposal - Students with a minimum 3.25 GPA may submit proposals by May 1st of the spring term of their junior year.  

Independent Study Registration Form (Undergraduate) - Apply to pursue an area of study not covered by the established curriculum. 

Independent Study Registration Form (Graduate) - Apply to pursue an area of study not covered by the established curriculum. 

Individual Study Registration Form (Undergraduate) - If a class is not offered during the term the student needs to take it, students may take the class on an individual study basis with instructor permission. Instructors work individually with students to complete the coursework.
