Declaration of Major/Minor/Certificate and Change of Advisor Flexible Curriculum Proposal - A student may design his or her own major with approval of this form. Must be submitted no sooner than the first term of the sophomore year and no later than the first term of the senior year. Grade Appeal Request Form - Students may appeal the grade assigned to them in a course. Submission of an appeal does not guarantee a grade change. Request for Incomplete Grade - Students may work with their instructors to contract for an incomplete grade in a course. Undergraduate Petition of Academic Policy - Students may request an exception to academic policy by submitting a petition to the Academic Affairs Committee. Submission of a petition does not guarantee approval.
