Bar Release Authorization - Law graduates authorize release of records to Boards of Law Examiners, State Bars, or equivalent licensing agencies. Course Substitution and Prior Approval - *NEW* - Use the program sheet associated with your major or minor to indicate a course substitution. The Course Substitution and Prior Approval form has been discontinued. Declaration of Major/Minor/Certificate and Change of Advisor Flexible Curriculum Proposal - An undergraduate student may design his or her own major with approval of this form. Must be submitted no sooner than the first term of the sophomore year and no later than the first term of the senior year. Grade Appeal Request Form - Students may appeal the grade assigned to them in a course. Submission of an appeal does not guarantee a grade change. Incomplete Grade Request - Students may work with their instructors to contract for an incomplete grade in a course. Name Change - Legal name and gender changes can be made, with appropriate supporting documentation, via this form. Petition of Academic Policy-Undergraduate - Students may request an exception to academic policy by submitting a petition. Submission of a petition does not guarantee approval. Transcript Request Registration Forms Undergraduate non-Hamline Visitor Registration Form - Visiting students use this form to register for undergraduate courses. ACTC Cross-Registration - Undergraduate form to request registration at one of Hamline's ACTC partner schools. Add/Drop/Withdrawal (Undergraduate) Add/Drop/Withdrawal (Graduate) Additional License Advanced Practicum Registration (OLD) - For students who were admitted to the additional license program prior to Fall 2015. Additional License Advanced Practicum Registration (NEW) - For students who were admitted to the additional license program in Fall 2015 or later. Apprentice Teaching Contract - Undergraduate students may earn a maximum of 4 credits toward graduation by assisting faculty with their teaching responsibilities. Apprentice teaching is graded on the Pass/No Pass scale. Audit Registration (Undergraduate) - Register for a course on a no-credit, ungraded basis for a flat fee. Collaborative Research Registration - Collaborative research projects offer undergraduate students the chance to pursue independent study in greater depth with a faculty member, often in conjunction with the professor’s own research. Departmental Honors Project Proposal - Students with a minimum 3.25 GPA may submit proposals by May 1st of the spring term of their junior year.
