VA Certification Request Form First Name Last Name Hamline ID Email Address Student Type (Select One) Undergraduate Graduate Post-Baccalaureate Certificate Visiting What type of benefit would you be using? (Post 9/11 GI Bill, Montgomery GI Bill, etc.) By submitting this form, I understand and acknowledge that: My enrollment will be submitted to the VA each term I am registered unless I request in writing not to be certified a specific term. Hamline must share my net tuition and fee charges for each term with the VA if I have Chapter 33 benefits. I must also submit my DD-214 and Certificate of Eligibility to Hamline’s School Certifying Official. I must apply to use my benefits with the Department of Veterans Affairs along with this request. In the event of a full or partial withdrawal, VA and Yellow Ribbon payments (including book and housing stipends) may be partially or fully prorated and I may owe money back. Classes completed with an “F” grade may cause VA payments (including book and housing stipends) to be partially or fully prorated and I may owe money back.
